They are opened via the file open command and via the file place command. If you open them with the place command then they are loaded as a layer; if opened via the file open then they are opened as a new document and you will be asked to specify a size and resolution for the file. Personally, the best way would be in Illustrator but they can be quickly created in PS as well. Create a circle using the tools on the toolbox and then stretch to create an oval. Duplicate. Rotate the first circle 30 degrees anti-clockwise. Rotate the other 30 degrees clockwise (the angle can vary). Align both with the bottom of both resulting in a smooth curve. Squeeze / transform or whatever to create a different look. Layer menu and combine layers and then select the circle tool in the toolbox. Top bar and combination options to merge components and edit menu define command if you want to create a CSH file but otherwise save as a PS PDF file via the save menu. Personally, prefer to create in Illustrator. Probably can do much the same in many apps such as Affinity Designer. You can create all kinds of different romantic work with different combinations of circles etc. You can then add sprinkles, gradients etc as required. Of course there the ones on this page as well as many other on the web (for free as well as paid for - always, as ever, read the license whether free or not to what you can do with them if you go that route)
You can use them in many more apps than just PS (I know I mention that excellent product perhaps too many times on this site) but it is a key tool. I created all the artworks in that app in the first place. You can open the PDF files via the open command as well as the place command and you can then edit the artwork in 1000s of ways such as adding additional paths to the item, changing the color of the PDF, adding effects as well as additional fills and strokes to the art, you can transform them, remove or add points, use them with third party plugins such as the powerful MirrorMe and VectorScribe from Astute Graphics as well as the Art Blue plugins from Amazon Canvas. You can then save the artwork as a new PDF or perhaps save them as a SVG or EPS file or a raster format file such as PNG
